Fill in multiple locations for existing providers based on available data

Once #336 is done, we should fill in the multiple locations for existing providers based on the data that we have. List of countries is stored in the registration spreadsheet. We can programmatically read the list of countries for each provider, and create a new location in the database for each country, referring back to the provider.
