Add Colemak-DH Wide cheatsheet, fix Colemak-DH cheatsheet

This PR adds a cheatsheet for Colemak-DH Wide¹ and fixes Colemak-DH cheatsheet to include the Angle Mod², which is canonically a part of Colemak-DH.

It is kind of hard to represent the way custom keyboard layouts like Colemak-DH look on ANSI vs ISO keyboards³ with the physical and the functional layouts being separated into different user-facing variables,⁴ as Colemak-DH and Colemak-DH Wide vary in key placement depending on the physical layout:
