I keep getting lost in this code (there's lots of different stream orderings, min / max, inclusive / exclusive going on).

This adds some docstrings and some inline comments, as well as a few other formatting changes. Should be reviewable commit-by-commit.

There is one functional change in the second commit and some code reordering, that should be a no-op.
