REDCap CSS Injector

REDCap CSS Injector allows REDCap administrators to inject CSS into surveys, data entry forms, and other project context pages.

Release 2.0.x introduces a breaking change in the settings that control where CSS is applied. See the Migration from version 1.X.X to 2.0.0 for guidance and upgrade procedures. This new granularity allows CSS to be applied to survey pages, data entry pages, and every other project page as needed. This release also bumps the framework version to 15 (@ChemiKyle, @saipavan10-git, @pbchase, #11, #16, #17)
