Add instructions for how to use rails-erd in a gem

I want to use rails-erd in a gem that defines various ActiveRecord models and migrations. The gem does not pull in rails or railties, so I cannot run rails g erd:install. Adding instructions on how to use rails-erd outside of rails would be very helpful.
