Email Opt-Out Template

To make the opting-out process easier for students whose professors would like to use Respondus, we need to add a page to our Resources tab that allows students to fill out a small form with their name, the professor's name, and the course code, and will subsequently produce the filled-out email template for students to copy + paste into their email client, with an optional button that opens up their email client with the filled-out email.
