Custom Action Adapters are "unchecked" when form is edited

In Plone 5, when a form is edited, the checkbox to enable a custom action adapter (assuming one has been created and is being used on the form), is automatically unchecked. In other words, it stays checked (enabled) until the form is edited; then it is automatically unchecked, so you have to recheck (re-enable) the custom action adapter before saving the form, every time the form is edited.
