Incorrect hyperlink to discussions in the question template for issues

A clear and concise description of what the bug is.

The template for questions suggests stating a new discussion but the url point at diffrent repository, which might cause people to open issues on the wrong project (just happend to me).

What do you expect to see?

A hyperlink to the discussions of this project: https://github.com/opensearch-project/opentelemetry-demo/discussions

What did you see instead?

A hyperlink to the discussions of a diffrent project: https://github.com/open-telemetry/opentelemetry-demo/discussions/

Reproduce

Issues -> New Issue -> Question -> Get Started:
