Several bug fixes and new tool #58

This PR fixes several bug, and implements new tools.

Highlights:

Previous tools compareCreate and CompareInsert are not bundled into MergeVersions to allow more complex merging.
JXFormToMySQL now checks for duplicated list_names in choices. Remove yes/no parameters and has an updated version of return codes. Please note that JXFormToMySQL requires for now an special version of PyXForm available here. This is because JXFormToMySQL requires the name of the list in choices and is not (at the moment) part of the official code. This will change once the PyXForm team accepts my PR :-)
