I'm not 100% sure, but it looks like multiple instances of the same issue ID cause it to appear multiple times in the changelog row.

We have PRs, commits, and branches all include out ticket number (e.g. ABC-1234) and when using the issuePattern setting, a merged PR is showing
Pull Request #1234 : ABC-1111 Added vital changes #ABC-1111 #ABC-1111 #ABC-1111
