it should be obvious to to the editor they shouldn't expect to see changes to the preview in real time if thats no enabled.

Ways this can be done

add button on preview won't appear
remove or other block actions on preview won't appear
a warning on the sidebar that changes aren't visible until saved? or maybe dismissable warning below like the lock warning?
