USDA library was contacted for a copy of the revised technical bulletin. The older bulletin is:

Scanning through the older bulletin, I do not see any functions published. My educated guess is the USFS approximated the site index functions reported in Hanson et al. (2003, #24). I will give McArdle the credit for the equations given in the Hanson report, but it is a bit of an edge case.
