Bulletproof file ingestion #2043

At the moment when a file fails to be ingested we simply log that information to our internal logs and nothing really happens. Instead we should provide a report to a users (after then ingest documents) that detail any files that do not get ingested and ideally a reason why the ingestion of that document failed.
