I'm a bit worried about screen readers when I see a
block of code filled with
s and other elements ーdon't you need a few aria-* attributes on them to tell the reader to not stop and tell each and every tag?

I just tried with NVDA. It seems to read

elements line by line, whether they are highlighted by prism or not.

There’s only one
element and that’s actually the way that's recommended by the HTML5 spec to mark up code blocks, not something Prism-specific. Spans have no semantics by default, and thus should not be acknowledged in any special way by screen readers.
