Why is util.encode used here?

Because the code can contain XML-like tags which will be parsed as HTML because I use env.element.innerHTML.
Also, it's what highlight does.

Ah, and this was handled previous here:
env.element.textContent = env.code;which uses textContent rather than innerHTML.

This PR fixes the bug described in #1706 (also includes a demo).
The problem was that the
insert-beforehook was not called ifgrammaris undefined.This also fixes that
callbackwas not called in that case and thatenvof thecompletehook did not have ahighlightedCodeproperty.All other hooks are unchanged. The
insert-beforehook is only used by the Command line plugin, so there should be no other side effects.
