Blank modal window when deleting a report card configuration

Issue Description:

When attempting to delete a report card configuration in the Grading > Report Card Setup page, the modal window that appears after clicking the blue x button is blank. This prevents the user from confirming the deletion of the configuration.

Steps to Reproduce:

1. Go to Grading > Report Card Setup.
2. Click the Add New Report Card button.
3. Enter a name for the report card and select the desired options.
4. Click the Save button.
5. Click the blue x button next to the report card you just created.
6. Observe that the modal window is blank.

Expected Behavior:

The modal window should display a message asking the user to confirm the deletion of the report card configuration.

Actual Behavior:

The modal window is blank.
