Require the ICD signature and attestation before the rest of the test, and don't allow it to be submitted until that part is filled in.

Include another link to the task instructions in the "make sure you've read the task instructions" part.

After the quiz, simply have them submit the HIT. Include some language saying that "We will review your responses and then contact you for more interactive dialogues." (Let's pay them $0.50 for filling out the form.)

Give people who pass this a qualification to do 2 dialogues. If they pass that we give them the general qualification to do many more dialogues.
